In this podcast, Tashau Asefaw, a Manager for Community Programs at Community Health Plan of Washington, or CHPW, discusses CHPW’s approach to locating and engaging hard-to-reach members during COVID-19. In 2021, Resources for Integrated Care launched an Integrated Care Community of Practice (ICCoP), a learning community of outreach and care coordination staff from 20 national and local health plans. These podcasts, excerpted from a December 2021 panel discussion webinar, discuss promising practices that emerged from the ICCoP and elsewhere for locating and engaging these members, especially in light of the COVID-19 pandemic. Panelists share challenges and innovative strategies for engaging members in care coordination, addressing social needs, and providing education and outreach related to the COVID-19 vaccine.
